The hospital does not keep copies of birth or death certificates on file. You must contact the Pennsylvania Department of Health for a copy of a legal birth or death certificate. The request can be made by telephone, U.S. mail or the Internet. There is a fee involved.

Patients have the right to copies of their medical records. In order to preserve patient privacy, a valid HIPAA authorization must be completed and signed by the patient/guardian. In the case of the deceased patient, the Executor of the decedent’s estate or in the absence of an executor, the next of kin along with a short certificate (from the County Register of Wills) or the person responsible for the disposition for the remains may consent.  Information submitted will be validated prior to releasing any medical information and/or additional information may be requested.

The authorization form must be completed in its entirety. Please be specific regarding the information to be released by indicating the date(s) of the specified admission type such as Inpatient, Emergency Department, Outpatient Surgery or Outpatient Diagnostic and check off appropriate records to be released/obtained.  An abstract is a composite of the record that is most helpful to our patients and contains the information that is sent to physicians for continuity of care. The abstract contains the discharge summary, history and physical, consultation reports, all operations, diagnostic and laboratory results.

Pennsylvania State law and/or 45 CFR §164.524(4)(i-iv) Access of individuals to protected health information have established the fees that hospitals and health care providers may charge for personal use.

  • After we receive your request, we will send you an invoice for the cost of the
    records you requested, if applicable.
  • Send payment as indicated on the invoice.
  • After we receive payment, records will be mailed or you can pick up the
    records with proper photo identification.

Please stop by the Visitors Information Desk at either location for assistance. The visitor’s desk is located near the front entrance, on the second floor at both WHS Washington Hospital and WHS Greene. The desk is staffed by hospital volunteers and employees who will assist you in locating family, friends and services. In the evening or off hours, you can ask for assistance at the emergency department desk.

WHS Washington Hospital

Between the hours of 9:00 p.m. and 5:30 a.m. all doors in the front of the hospital are locked. You can enter through the Emergency Department entrance between those hours if needed.

Visitors are encouraged to park on the top two levels of the visitors parking garage, located in the front of the WHS Washington Hospital.

Emergency patients and/or visitors may park in the Emergency parking lot located in the back of the hospital. Just follow the signs for the emergency room, once you are on the campus.

Parking gates will dispense a ticket to each vehicle upon pulling up to the gate. If paying with cash, visitors should take the ticket with them and pay before leaving the hospital at one of the kiosks located in the ED, Lobby or UPMC Cancer Center. If paying with credit card, visitors will be able to pay directly at the exit gate after inserting their parking ticket.

  • 0-70 minutes: Free
  • 71 minutes-2 hours: $1
  • 2-3 hours: $2
  • 3-4 hours: $3
  • 4+ hours: $4
  • Daily max/Lost ticket: $4

Metered parking operated by the City of Washington is available on the streets adjacent to the hospital.

WHS Greene

Free parking is located in front of the WHS Greene facility.

Outpatient Facilities

Most of our outpatient facilities provide free parking areas. If this is a concern, please call the facility before your appointment to confirm.

If you are visiting a patient at WHS Washington Hospital or WHS Greene, please do not use the bathroom that is located inside a patient’s room. They are reserved for the patient in order to protect their health. Public restrooms are located throughout both hospitals. Please ask a WHS staff member for the closest restroom.

WHS Washington Hospital: Pastoral Care is available 24 hours a day, seven days a week. The director of Pastoral Care works in conjunction with The Greater Washington Ministerium, Catholic priests, and Eucharistic ministers. Chaplains can be contacted by calling the operator at 724-225-7000 or “0 ” in the hospital. A chapel is locational on the 2nd floor, near the main lobby.

WHS Greene: Pastoral Care is available. Please ask your nurse for additional details. A chapel is locational on the 4th floor.

WHS Education Department offers Heartsaver CPR and First Aid classes for the community twice a year. Contact the Education Department at (724) 229-1744 for the next scheduled class and details.
