We participate in most healthcare plans including Medicaid. Our participation allows for the highest benefit coverage for our clients; however this does not guarantee coverage. We advise you to contact your insurance company regarding your specific benefits prior to initiating therapy services.
An important part of the intake and assessment process is getting accurate information from you. It is important that we receive these forms prior to your first appointment so that we can plan the best possible assessment for your child. (Due to confidentiality rules, we cannot accept this health protected information via email)
Upon scheduling an initial evaluation, you will be asked to provide a current valid email address. Our office staff will send a link to your email that will provide you access to our patient portal. In the portal you will complete a pediatric medical history prior to the date of your child’s evaluation. You will also electronically acknowledge patient responsibilities form within the portal.
For your convenience, you may download and print the forms below prior to your appointment. To view a form, click on the name of the file to open it (Adobe Acrobat Reader needed). You will then be able to print the form to complete.
